Effect of custom-made foot orthoses versus placebo in patients with rheumatoid arthritis: randomized clinical trial. Pilot study

ABSTRACT

Introduction: Foot disorders are one of the biggest problems in people with rheumatoid arthritis (RA). The prescription of different types of foot orthoses is a common practice in these patients with pain in the feet. Although its effectiveness has not been rigorously evaluated.
Patients and methods: This is a randomized clinical trial. At the beginning of the study, the analog visual pain scale and the foot function index were administered. Participants were randomly assigned to one of the study groups: Experimental Group A (custom-made foot orthoses for pronation control) and Control Group B (plantar support of non-adapted buffer material). After three months, the same questionnaires were administered as at the beginning.
Results: The sample consists of a total of 17 patients with RA; 58.8 % Group A (experimental) and 41.2 % Group B (control). Comparing the means before and after the intervention in the different scales, in the control group there were no statistically significant differences. On the contrary, in the experimental group there is a statistically significant decrease in the visual analog scale and in different sections of the foot function index.
Conclusions: According to the results of this study, custom-made foot orthoses in patients with RA could improve pain and foot function.



RESUMEN

Introducción: Las patologías podológicas son uno de los mayores problemas en personas con artritis reumatoide. La prescripción de distintos tipos de ortesis plantares es una práctica común en estos pacientes con dolor en los pies, aunque su eficacia no se ha evaluado rigurosamente.
Pacientes y métodos: Se trata de un ensayo clínico aleatorio. Al principio del estudio se administra, la escala visual analógica del dolor, y el foot function index. Se asignaron aleatoriamente a uno de los grupos de estudio: grupo A experimental (soporte plantar a medida para control de pronación) y grupo B control (soporte plantar de material amortiguador sin adaptar). A los tres meses se le administran los mismos cuestionarios que al principio.
Resultados: La muestra está formada por un total de 17 pacientes con AR; el 58.8 % el grupo A (experimental) y el 41.2 % el grupo B (control). A comparar las medias antes y después de la intervención en las distintas escalas, en el grupo control no existen diferencias estadísticamente significativas. Por el contrario, en el grupo experimental existe una disminución estadísticamente significativa en la escala escala visual analógica y en distintos apartados del foot function index.
Conclusiones: Según los resultados de este estudio, las ortesis plantares realizadas a medida en pacientes con AR podrían mejorar el dolor y la función del pie.
